Welcome to the website of Revive Men’s Hair Clinic LLC (“Revive,” “Revive Men’s Hair Clinic,” “we,” “our,” or “us”). By accessing or using this website, requesting information, scheduling a consultation, booking an appointment, submitting a deposit, providing a payment method, signing treatment documents, receiving services, or otherwise engaging with Revive, you acknowledge that you have read, understood, and agreed to these Terms & Conditions.


These Terms & Conditions are incorporated into and made part of any estimate, invoice, treatment plan, consent form, payment authorization, financing arrangement, appointment confirmation, intake form, and other agreement between you and Revive, unless expressly stated otherwise in a written agreement signed by Revive.


1. USE OF THIS WEBSITE


The content provided on this website is for educational and informational purposes only. Nothing on this website should be interpreted as medical advice, a diagnosis, a treatment recommendation, or a guarantee of eligibility, candidacy, outcome, or result.


For all medical concerns, you should consult with a licensed healthcare provider. Use of this website does not create a provider-patient relationship with Revive or any of its owners, providers, employees, contractors, or representatives.


Unauthorized use of this website, including attempts to copy, scrape, interfere with, damage, reverse engineer, misuse, or exploit any website content, systems, forms, media, or functionality, may give rise to claims for damages, injunctive relief, attorney’s fees, costs, and any other remedies available under applicable law.


2. ELIGIBILITY


By using this website, requesting information, booking a consultation, or scheduling services, you represent that you are at least 18 years of age and legally permitted to receive medical care in the State of Nevada under applicable federal, state, and local law.


Revive may decline, postpone, cancel, or discontinue services if you are not an appropriate medical candidate, fail to provide accurate information, fail to follow pre-operative or post-operative instructions, fail to complete required documentation, fail to obtain required medical clearance, fail to pay amounts owed, or otherwise fail to comply with Revive’s policies.


3. MEDICAL SERVICES DISCLAIMER


All treatments and services, including but not limited to FUE, NeoGraft®, PRP, and WJ-MSC / “stem cell” procedures, require evaluation, candidacy determination, informed consent, and approval by a licensed medical provider.


Results vary by individual. Revive does not guarantee any specific result, timeline, density, hair count, cosmetic outcome, permanence, degree of improvement, or patient satisfaction. Before-and-after photos, testimonials, case studies, marketing content, or educational materials are examples only and do not guarantee or imply that you will receive the same or similar results.


Your final treatment plan, candidacy, graft count, clinical recommendations, and procedure eligibility may change based on your medical history, donor area, scalp condition, medications, compliance with instructions, provider evaluation, and other individualized factors.


4. INVESTIGATIONAL PROCEDURE NOTICE FOR WJ-MSC / “STEM CELL” PROCEDURES


Information on this website about WJ-MSC, “stem cell,” or regenerative procedures is provided for general informational purposes only and is not medical advice. WJ-MSC procedures offered by Revive for hair restoration are investigational and are not FDA-approved for the treatment of hair loss.


Revive does not guarantee any specific outcome from WJ-MSC procedures, including but not limited to hair density, hair count, growth rate, thickness, permanence, or prevention of future hair loss. Individual results vary.


Any decision to proceed with treatment will be made only after consultation with a licensed provider and completion of appropriate medical consent documentation addressing risks, benefits, alternatives, eligibility, and limitations.


5. TELEHEALTH & VIRTUAL CONSULTATIONS


Virtual consultations are provided for preliminary educational purposes, general planning, and convenience only. A virtual consultation is not a substitute for an in-person medical evaluation and does not guarantee candidacy, pricing, graft count, eligibility, procedure approval, or final treatment recommendations.


Final treatment eligibility always requires provider review, completion of required documentation, and, when applicable, an in-person evaluation and clearance.


6. APPOINTMENTS, DEPOSITS & PROCEDURE SCHEDULING


A $500 deposit is required to reserve any hair restoration service or procedure date, unless Revive expressly states otherwise in writing.


The deposit is applied toward the total cost of your scheduled service or procedure, provided you attend the appointment, comply with all applicable requirements, and pay all remaining amounts due. Deposits are non-refundable except where Revive expressly approves a refund in writing or where applicable law requires otherwise.


Procedure dates require substantial advance preparation, including provider scheduling, clinical team scheduling, appointment blocking, room preparation, administrative processing, treatment planning, equipment allocation, supply preparation, opportunity cost, and the inability to schedule another patient for the reserved date. Because these damages are difficult to calculate precisely, the cancellation, rescheduling, no-show, late-arrival, payment, and patient-caused inability-to-proceed terms below are intended as a reasonable liquidated-damages and financial-responsibility policy, not as a penalty.


7. PAYMENT IN FULL, CLEARED FUNDS, FINANCING & OUTSTANDING BALANCES


Unless Revive expressly agrees otherwise in writing, payment in full or approved third-party financing must be completed no later than five (5) business days before the scheduled procedure date.


In all events, all outstanding balances for any scheduled hair restoration service or procedure must be paid in full, in cleared funds, no later than 8:00 AM Pacific Time on the scheduled date of hair restoration.


No hair restoration service or procedure will begin, proceed, or be performed unless the patient’s balance has been paid in full and Revive has confirmed receipt of cleared funds.


Same-day payment may be accepted only by debit card, credit card, cashier’s check, or cash, unless Revive expressly approves another payment method in writing. Credit card payments may be subject to an additional transaction fee as described in these Terms & Conditions.


To the fullest extent permitted by applicable law, card-network rules, and Revive’s payment processor requirements, payments made by credit card may incur an additional transaction fee of up to three percent (3%) of the transaction amount.


This transaction fee is intended to offset Revive’s cost of accepting credit card payments and will not exceed the amount permitted by applicable law, card-network rules, or Revive’s actual cost of acceptance, whichever is lower.


Any applicable credit card transaction fee will be disclosed before payment and may be listed separately on the invoice, payment page, receipt, or transaction record.


Credit card transaction fees are non-refundable to the fullest extent permitted by law, including in the event of cancellation, rescheduling, refund, chargeback, payment dispute, declined payment, reversed payment, patient-caused inability to proceed, or failure to attend the scheduled appointment.


Revive may offer payment methods that do not incur a credit card transaction fee, including cash, cashier’s check, debit card, or other approved payment methods, subject to Revive’s payment deadlines and cleared-funds requirements.


If the patient uses or intends to use any payment method other than debit card, credit card, cashier’s check, or cash, including but not limited to ACH, wire transfer, personal check, business check, third-party payment platform, financing, or any other delayed, pending, or non-instant payment method, payment must be made in full and received as cleared funds at least five (5) business days before the original scheduled hair restoration date.


Pending payments, unverified payments, delayed transfers, unapproved financing, financing that has not funded, checks that have not cleared, disputed payments, reversed payments, payment holds, processor delays, or payment attempts that have not been fully received by Revive do not constitute payment in full.


If the balance is not paid in full by 8:00 AM Pacific Time on the scheduled date of hair restoration, Revive may refuse to proceed with the hair restoration service or procedure, retain any amounts paid, charge any outstanding balance to any authorized payment method on file, and treat the failure to pay as a patient-caused cancellation or inability to proceed. Any decision to reschedule, credit, apply prior payments, or make an exception is within the sole discretion of Revive Men’s Hair Clinic LLC.


Third-party financing is subject to the terms, conditions, underwriting, approval, cancellation, refund, dispute, and repayment rules of the applicable financing provider. Revive is not responsible for financing denials, financing delays, declined financing, chargebacks, payment reversals, repayment obligations, interest, late fees, or disputes between you and any third-party financing provider.


If financing is denied, reversed, cancelled, charged back, disputed, clawed back, not funded, partially funded, or otherwise not received by Revive, you remain personally responsible for the full amount owed to Revive.


8. CARD-ON-FILE, STORED PAYMENT CREDENTIAL & AUTO-PAYMENT AUTHORIZATION


By providing a credit card, debit card, bank account, financing account, payment link, digital wallet, or other payment method to Revive, or by allowing another person to provide a payment method on your behalf, you authorize Revive and its payment processors to securely store that payment method and charge it for any amounts you owe to Revive.


This authorization includes, but is not limited to, deposits, procedure balances, treatment balances, consultation fees, administrative fees, cancellation fees, rescheduling fees, no-show fees, late-arrival charges, patient-caused inability-to-proceed balances, liquidated damages, declined-payment fees, chargeback-related amounts, collection costs, attorney’s fees, interest, and any other amounts due under these Terms & Conditions, any invoice, any signed payment authorization, any treatment plan, or any other agreement with Revive.


You authorize Revive to charge any payment method you provide, previously used, placed on file, or authorized for use with Revive, including replacement cards, updated cards, or payment credentials made available through a card-network account updater or payment processor, to the fullest extent permitted by law and payment-network rules.


You authorize Revive to process charges in one or more transactions and, when necessary, to split a balance into multiple charges. You agree that Revive is not required to obtain a new authorization before each charge for amounts owed, provided the charge is consistent with these Terms & Conditions, a signed authorization, an invoice, treatment plan, or other written or electronic agreement.


If you provide a payment method that belongs to another person or entity, you represent and warrant that you are authorized to use that payment method for amounts owed to Revive. You agree to indemnify, defend, and hold harmless Revive from any claim, dispute, chargeback, reversal, or loss arising from your use of a payment method that you were not authorized to use.


You may revoke authorization for future charges by providing written notice to Revive; however, revocation of payment authorization does not cancel your appointment, waive any balance owed, release you from financial responsibility, or limit Revive’s right to pursue collection of amounts owed.


9. CANCELLATION, RESCHEDULING, NO-SHOW & LATE-ARRIVAL POLICY


Cancellations or rescheduling requests made within five (5) business days of the scheduled hair restoration date, no-shows, or arrivals more than forty-five (45) minutes late may result in forfeiture of any amounts paid, including deposits and partial payments.


“Business days” means Monday through Friday, excluding federal holidays, Nevada state holidays, and days on which Revive is closed.


In addition, if you cancel or request to reschedule within five (5) business days of the scheduled hair restoration date, fail to appear, are more than forty-five (45) minutes late, fail to pay all balances in full by 8:00 AM Pacific Time on the scheduled date of hair restoration, fail to complete required documents, fail to follow pre-operative instructions, fail to obtain required clearance, arrive in a condition that prevents the procedure from safely proceeding, or otherwise cause the procedure not to occur as scheduled, Revive may charge any outstanding balance due for the scheduled service or procedure to any authorized payment method on file.


If no payment method is available, declined, expired, disputed, blocked, or otherwise unavailable, the outstanding balance is due by 5:00 PM Pacific Time on the original scheduled date of the hair restoration procedure.


Revive reserves the right to determine, in its sole discretion, whether a cancellation, rescheduling request, no-show, late arrival, nonpayment, incomplete documentation, failed clearance, medication issue, medical noncompliance, intoxication, impairment, alcohol use, substance use, or other patient-caused issue triggers this policy.


10. FITNESS FOR PROCEDURE, ALCOHOL, INTOXICATION & SUBSTANCE USE


Patient safety is a condition of all hair restoration services. Each patient must arrive for the scheduled hair restoration procedure medically appropriate, alert, cooperative, unimpaired, and fit to proceed in the clinical judgment of Revive and its licensed providers.


Patients may not arrive intoxicated, impaired, under the influence of alcohol, under the influence of recreational drugs, under the influence of illegal substances, or otherwise in a condition that Revive determines may create a safety concern, clinical concern, compliance issue, consent issue, or inability to proceed.


Patients must not consume alcohol within the twenty-four (24) hour period preceding 8:00 AM Pacific Time on the scheduled date of hair restoration.


Patients must fully and accurately disclose all medications, controlled substances, supplements, alcohol use, recreational drug use, and any other substance use requested by Revive or its providers. Properly prescribed medications must still be disclosed and may require provider review before the procedure may proceed.


If Revive determines, in its sole clinical and business discretion, that the patient is intoxicated, impaired, under the influence, has consumed alcohol within the prohibited time period, has failed to disclose relevant medication or substance use, presents with unsafe symptoms, presents with unsafe vital signs, cannot safely consent, cannot safely participate, or otherwise cannot safely proceed with the scheduled hair restoration service or procedure, Revive may cancel, postpone, refuse, or stop the procedure.


If the procedure cannot proceed for any of these reasons, the situation will be treated as a patient-caused inability to proceed, late cancellation, or no-show. All balances remain due in full. Revive may retain any amounts paid and may charge any outstanding balance to any authorized payment method on file. Any reschedule, credit, partial credit, exception, or application of prior payments to a future date is not guaranteed and is within the sole discretion of Revive Men’s Hair Clinic LLC.


11. RESCHEDULING OUTSIDE THE FIVE-BUSINESS-DAY WINDOW


If you request to reschedule more than five (5) business days before the scheduled hair restoration date, Revive may, in its sole discretion, allow the deposit to be applied to a new procedure date.


Revive is not required to offer unlimited rescheduling. Additional rescheduling may require an additional deposit, updated pricing, updated medical clearance, updated consent forms, updated provider approval, or payment in full before the new appointment date.


If pricing, clinical recommendations, graft count, service availability, provider availability, or treatment recommendations change before the rescheduled date, Revive may require updated approval, updated documentation, and updated payment terms.


12. PROVIDER-INITIATED CHANGES, MEDICAL DETERMINATIONS & EXCEPTIONS


Revive may cancel, postpone, reschedule, modify, or decline treatment for medical, operational, provider-scheduling, safety, compliance, staffing, equipment, emergency, or other business reasons.


If Revive initiates a cancellation or rescheduling for reasons not caused by your conduct, nonpayment, noncompliance, incomplete paperwork, inaccurate disclosures, intoxication, impairment, failure to follow instructions, or failure to obtain required clearance, Revive may offer a rescheduled appointment, apply amounts paid to a future service, or issue a refund for unperformed services, as appropriate.


Exceptions for documented medical emergencies, airline cancellations, severe weather events, provider-initiated changes, duplicate payments, or other extraordinary circumstances may be reviewed by Revive on a case-by-case basis. Documentation may be required. Revive is not obligated to approve any exception unless required by applicable law.


13. REFUND POLICY


All deposits are non-refundable unless Revive expressly approves a refund in writing or applicable law requires otherwise.


No refunds are provided for services already performed, partially performed, initiated, prepared for, or made unavailable to other patients because of your booking, late cancellation, no-show, late arrival, nonpayment, noncompliance, intoxication, impairment, alcohol use, substance use, failure to obtain required clearance, or failure to proceed.


Refunds, when approved, may be reduced by deposits, administrative costs, payment-processing fees, chargeback fees, financing fees, clinical preparation costs, supplies ordered, provider time, staff time, and other costs incurred by Revive, to the fullest extent permitted by law.


Revive does not issue refunds based on dissatisfaction with results, failure to achieve a desired cosmetic outcome, failure to follow pre-operative or post-operative instructions, normal variability in healing or growth, or expectations not expressly guaranteed in writing by Revive.


14. DECLINED PAYMENTS, CHARGEBACKS & PAYMENT DISPUTES


If any payment is declined, reversed, charged back, disputed, clawed back, returned, held, delayed, or otherwise not received by Revive, you remain responsible for the full amount owed.


You agree to contact Revive in writing and allow Revive a reasonable opportunity to investigate and resolve any billing concern before initiating a chargeback, payment dispute, financing dispute, or other reversal request.


Initiating a chargeback or payment dispute for an amount you authorized, agreed to pay, or owe under these Terms & Conditions does not cancel your debt. Revive may provide these Terms & Conditions, signed authorizations, invoices, appointment confirmations, intake records, communications, and other documentation to banks, card networks, financing providers, processors, collection agencies, attorneys, courts, arbitrators, or other appropriate parties to dispute a chargeback, collect amounts owed, or protect Revive’s rights.


You agree to reimburse Revive for chargeback fees, returned-payment fees, processor fees, collection costs, attorney’s fees, court costs, arbitration fees, and other expenses arising from any improper, unsuccessful, or unsupported payment dispute, to the fullest extent permitted by law.


15. COLLECTIONS, INTEREST, FEES & ATTORNEY’S FEES


Any unpaid amount may accrue interest at the lesser of 1.5% per month or the maximum rate permitted by applicable law, beginning on the date the amount becomes due.


Revive may apply payments first to fees, costs, interest, prior balances, or the oldest outstanding amounts before applying payments to current balances.


If Revive refers any unpaid balance to collections, an attorney, a court, arbitration, or another recovery process, you agree to pay all reasonable collection costs, attorney’s fees, court costs, arbitration fees, service fees, filing fees, processor fees, and other expenses incurred by Revive in collecting amounts owed or enforcing these Terms & Conditions, to the fullest extent permitted by law.


16. PRICING, ESTIMATES & TREATMENT PLANS


Pricing for treatments is provided transparently but may vary based on consultation findings, graft count, procedure type, provider recommendation, candidacy, promotions, financing fees, patient goals, medical history, and other individualized factors.


Any quote, estimate, consultation discussion, advertisement, or pricing representation is not final unless confirmed in a written invoice, treatment plan, or agreement issued by Revive.


Revive may update pricing, promotions, discounts, financing options, service offerings, deposit requirements, and payment policies at any time. Unless Revive expressly agrees otherwise in writing, updated pricing and policies may apply to future consultations, rescheduled appointments, modified procedures, additional services, or new bookings.


17. PATIENT RESPONSIBILITIES


You agree to provide accurate, complete, and current information regarding your identity, contact information, medical history, medications, allergies, prior procedures, health conditions, lifestyle factors, and any other information requested by Revive.


You agree to follow all pre-operative and post-operative instructions provided by Revive. Failure to follow instructions may affect your candidacy, safety, healing, results, eligibility, and financial obligations.


You are responsible for attending scheduled appointments on time, completing required paperwork, obtaining required clearance, arranging transportation when needed, complying with medication instructions, and paying all amounts owed when due.


You are responsible for arriving medically appropriate, unimpaired, and fit to proceed, and you must comply with Revive’s Fitness for Procedure, Alcohol, Intoxication & Substance Use policy.


18. NO GUARANTEE OF RESULTS


Revive makes no guarantee, warranty, or promise regarding any medical, cosmetic, aesthetic, emotional, financial, psychological, or other outcome.


Hair restoration outcomes depend on many individualized factors, including but not limited to donor availability, graft survival, hair characteristics, health status, medications, age, genetics, scalp condition, compliance with instructions, healing response, and future hair loss.


You acknowledge that additional procedures, maintenance treatments, medications, therapies, or follow-up care may be recommended or desired, and that additional costs may apply.


19. INTELLECTUAL PROPERTY


All website content, including text, graphics, logos, designs, photographs, videos, media, layouts, trademarks, trade names, service marks, and other materials, is owned by Revive or licensed third parties.


You may not copy, reproduce, distribute, modify, scrape, publish, display, sell, exploit, or create derivative works from any website content without Revive’s prior written consent.


20. TRADEMARKS


NeoGraft® is a registered trademark of Venus Concept Inc. Revive Men’s Hair Clinic is not affiliated with, sponsored by, or endorsed by Venus Concept Inc.


All other trademarks, trade names, product names, and logos are the property of their respective owners.

You may opt out of marketing communications where applicable, but opting out of marketing communications does not prevent Revive from sending transactional, appointment-related, billing-related, legal, or healthcare-related communications.


25. PRIVACY & HIPAA


Your use of this website and Revive’s services is also governed by Revive’s Privacy Policy and HIPAA Notice of Privacy Practices, which are available on Revive’s website and incorporated by reference into these Terms & Conditions.


26. ELECTRONIC SIGNATURES & RECORDS


You agree that electronic signatures, clickwrap agreements, checkbox authorizations, typed names, digital acknowledgments, online forms, payment authorizations, text confirmations, email confirmations, and electronically stored records may be used to establish your agreement, consent, authorization, and acceptance of these Terms & Conditions and related documents.


You agree that electronic records may be used as evidence of your agreement and authorization to the fullest extent permitted by law.


27. LIMITATION OF LIABILITY


To the fullest extent permitted by law, Revive, its owners, providers, employees, contractors, representatives, affiliates, vendors, and agents are not liable for any indirect, incidental, consequential, special, punitive, exemplary, or speculative damages arising from or related to your use of this website, reliance on website content, third-party links, payment issues, scheduling issues, financing issues, treatment expectations, or services, except where liability cannot legally be limited.


Nothing in these Terms & Conditions is intended to limit rights that cannot be limited under applicable law.


28. NO WARRANTY OF WEBSITE AVAILABILITY


Revive does not warrant that this website will always be available, uninterrupted, error-free, secure, accurate, complete, updated, or free from viruses or harmful components.


Revive may update, suspend, modify, remove, or discontinue website content, forms, functionality, promotions, or services at any time without notice.


29. INDEMNIFICATION


You agree to indemnify, defend, and hold harmless Revive, its owners, providers, employees, contractors, representatives, affiliates, vendors, and agents from and against any claims, demands, disputes, liabilities, damages, losses, chargebacks, reversals, penalties, costs, attorney’s fees, and expenses arising out of or related to:

(d) your failure to pay amounts owed;

(e) your unauthorized use of a payment method;

(f) your failure to follow instructions;

(g) your improper chargeback or payment dispute;

(h) your intoxication, impairment, alcohol use, substance use, or failure to disclose relevant medical or substance-use information;

(i) your violation of applicable law; or

(j) any claim brought by a third party relating to your conduct, authorization, payment method, or account.


30. GOVERNING LAW, VENUE & JURISDICTION


These Terms & Conditions are governed by the laws of the State of Nevada and applicable federal law, without regard to conflict-of-law rules.


To the fullest extent permitted by law, any claim, dispute, collection action, payment dispute, or legal proceeding arising out of or related to these Terms & Conditions, Revive’s website, payment obligations, scheduling, services, or relationship with Revive shall be brought exclusively in a court of competent jurisdiction located in Clark County, Nevada.


You consent to personal jurisdiction and venue in Clark County, Nevada.


31. INDIVIDUAL CLAIMS ONLY; NO CLASS ACTIONS


To the fullest extent permitted by law, you and Revive agree that any claim, dispute, or proceeding must be brought only on an individual basis and not as a plaintiff, claimant, class member, or representative in any class action, collective action, private attorney general action, consolidated action, or representative proceeding.


32. SEVERABILITY


If any provision of these Terms & Conditions is found to be invalid, unlawful, void, or unenforceable, the remaining provisions will remain in full force and effect. The invalid or unenforceable provision will be modified to the minimum extent necessary to make it valid and enforceable, if permitted by law.


33. NO WAIVER


Revive’s failure to enforce any provision of these Terms & Conditions does not constitute a waiver of Revive’s right to enforce that provision or any other provision in the future.


34. ENTIRE AGREEMENT; CONFLICTS


These Terms & Conditions, together with any signed treatment documents, informed consents, financial responsibility agreements, payment authorizations, invoices, treatment plans, Privacy Policy, and HIPAA Notice of Privacy Practices, constitute the agreement between you and Revive regarding the subject matter addressed.


If there is a conflict between these Terms & Conditions and a later written agreement signed or electronically accepted by you and Revive, the later written agreement controls as to that specific conflict.


35. UPDATES TO TERMS


Revive reserves the right to update or modify these Terms & Conditions at any time. Updates will be posted on this page with a revised effective date.
