Welcome to the website of Revive Men’s Hair Clinic LLC (“Revive,” “we,” “our,” or “us”). By accessing or using this website, scheduling consultations, booking services, communicating with our clinic, or otherwise engaging with our services, you agree to be bound by these Terms & Conditions. If you do not agree with these Terms, please do not use this website or our services.

1. Use of This Website

The information provided on this website is intended solely for educational and informational purposes.

Nothing contained on this website constitutes medical advice, diagnosis, or treatment and should not be relied upon as a substitute for consultation with a qualified healthcare provider.

Always seek the advice of a licensed physician regarding any medical condition or treatment.

Information on this website is provided for general educational purposes only. Nothing contained herein creates a physician-patient relationship until a patient has been formally accepted for care by Revive Men’s Hair Clinic and all required consent documents have been completed.

2. Eligibility

3. Medical Services Disclaimer

Treatment recommendations are individualized based upon each patient’s medical history, examination, donor characteristics, treatment goals, and provider judgment.

Revive Men’s Hair Clinic makes no guarantees or warranties regarding:

  • Hair growth
  • Hair density
  • Cosmetic appearance
  • Graft survival
  • Treatment longevity
  • Recovery timelines
  • Future hair loss progression

Investigational Procedure Notice (WJ-MSC / “Stem Cell”)

Information provided regarding Wharton’s Jelly Mesenchymal Stem Cell (WJ-MSC) procedures is intended solely for educational purposes.

Procedures utilizing WJ-MSC for hair restoration are investigational and are not FDA-approved for the treatment of hair loss.

No statements made on this website have been evaluated by the U.S. Food and Drug Administration for the treatment of hair loss.

Any decision to proceed with treatment will occur only after consultation with a licensed provider and completion of informed consent documents discussing:

  • Risks

3A. Telehealth & Virtual Consultations

Virtual consultations are intended for preliminary education, discussion of treatment options, and planning purposes only.

Virtual consultations do not establish a physician-patient relationship for treatment, do not replace an in-person medical examination, and should not be relied upon for diagnosis or emergency medical care.

Final candidacy for treatment always requires an in-person evaluation and provider approval.

3B. Right to Refuse or Discontinue Treatment

Revive Men’s Hair Clinic reserves the right to refuse, postpone, discontinue, or terminate any consultation, treatment, or procedure at any time if, in the professional judgment of Revive or its Medical Director, doing so is necessary to protect the health, safety, or well-being of the patient, staff, or others.

Reasons may include, but are not limited to:

  • Failure to obtain or maintain appropriate medical clearance.
  • Failure to truthfully disclose medical history, medications, allergies, or other relevant health information.
  • Refusal to follow physician recommendations or medical instructions.
  • Abusive, threatening, harassing, disruptive, or inappropriate behavior toward physicians, staff, or other patients.
  • Intoxication or impairment due to alcohol, illegal drugs, cannabis, prescription medications, or other substances that may impair judgment or safely permit treatment.
  • Any condition or circumstance that, in the professional judgment of Revive, could compromise patient safety or treatment outcomes.

If treatment is discontinued for any of the reasons above, applicable cancellation policies, fees, and payment obligations contained within these Terms & Conditions shall remain in effect.

3C. Medical Photography

As part of evaluation, treatment planning, documentation, follow-up, and outcome assessment, Revive Men’s Hair Clinic routinely obtains medical photographs and videos before, during, and after treatment.

These photographs become part of the patient’s confidential medical record and may be used for:

  • Medical documentation.
  • Treatment planning.
  • Clinical evaluation.
  • Quality assurance.
  • Follow-up comparisons.

No patient photographs, videos, or identifying information will be used for advertising, marketing, educational materials, websites, or social media without the patient’s separate written authorization.

4. Appointments, Deposits, Cancellations & Payments

Scheduling Deposits

To reserve your appointment, the following scheduling deposits are required:

Scheduling deposits will be applied toward the cost of your scheduled treatment or procedure.

Deposit Forfeiture

Scheduling deposits may be forfeited if:

  • Cancellation or rescheduling occurs fewer than ten (10) business days before the scheduled appointment or procedure.
  • The patient fails to appear for the scheduled appointment or procedure (no-show).
  • The patient arrives more than forty-five (45) minutes late and the appointment or procedure cannot reasonably be completed as scheduled.

Full Treatment or Procedure Charges

Because Revive Men’s Hair Clinic reserves dedicated physician, clinical staff, operating facilities, equipment, and appointment time specifically for each scheduled patient, cancellations on short notice or failure to appear may result in significant scheduling disruptions and financial loss.

Accordingly, Revive Men’s Hair Clinic LLC reserves the right to charge the payment method on file, including any approved financing account, for up to the full cost of the scheduled consultation, treatment, or procedure if any of the following occur:

  • Cancellation or rescheduling occurs within five (5) business days of the scheduled appointment or procedure.
  • The patient fails to appear for the scheduled appointment or procedure (no-show).
  • The patient arrives more than forty-five (45) minutes late and the appointment or procedure cannot reasonably be completed as scheduled.
  • The treatment, or procedure is canceled, postponed, or terminated because the patient engages in unsafe, threatening, abusive, disruptive, intoxicated, or otherwise inappropriate behavior that, in the professional judgment of Revive Men’s Hair Clinic or its Medical Director, compromises the safety of the patient, staff, or others or prevents the procedure from being safely performed.

By scheduling an appointment or procedure and providing a credit card, debit card, ACH authorization, financing account, or other payment method, the patient expressly authorizes Revive Men’s Hair Clinic LLC to charge the payment method on file for any applicable cancellation charges, no-show charges, late-arrival charges, outstanding balances, or the full treatment or procedure fee, as permitted by these Terms & Conditions and applicable law.

If the payment method on file is declined, rejected, expires, or otherwise fails to process, the patient remains fully responsible for all amounts due. Revive Men’s Hair Clinic reserves the right to pursue any lawful means of collecting unpaid balances, including collection agencies, legal action, recovery of reasonable collection costs, attorneys’ fees where permitted by law, and any other remedies available under applicable law.

Revive Men’s Hair Clinic may, at its sole discretion, waive or reduce any cancellation, no-show, or late-arrival charges based upon documented extraordinary circumstances.

Exceptions

Exceptions may be considered at Revive Men’s Hair Clinic’s sole discretion for documented extraordinary circumstances, including:

  • Medical emergencies
  • Airline cancellations
  • Severe weather events
  • Provider-initiated schedule changes
  • Duplicate payments
  • Other documented extraordinary circumstances

Payment Requirements

Unless otherwise approved in writing by Revive Men’s Hair Clinic:

  • Payment in full or approved financing arrangements must be completed no later than five (5) business days before the scheduled procedure.
  • ACH payments must be received no later than five (5) business days before the scheduled procedure to allow sufficient processing time.
  • Credit card payments are subject to a 2.99% processing fee.
  • Cashier’s checks must be made payable to Revive Men’s Hair Clinic LLC.
  • Cash and debit card payments are accepted with no processing fee.
  • Other forms of payment may be accepted at Revive Men’s Hair Clinic’s discretion.
  • All outstanding balances must be paid in full before any consultation, treatment, or procedure commences.

Revive Men’s Hair Clinic reserves the right to refuse, reschedule, postpone, or cancel any consultation, treatment, or procedure if required deposits, payments, financing arrangements, completed documentation, or outstanding balances are not received or satisfied within the required timeframes.

5. Pricing, Financing & Patient Satisfaction Commitment

5.1 Pricing

Pricing may change at any time prior to scheduling or execution of a treatment agreement.

All treatment recommendations, graft estimates, pricing, and treatment plans are individualized based upon each patient’s consultation, medical evaluation, donor characteristics, treatment goals, and provider recommendations.

Final treatment recommendations and pricing may change following the physician’s evaluation or changes in the patient’s medical condition.

5.2 Personalized Quotes, Promotions & Special Offers

All personalized treatment plans, written quotes, promotional pricing, discounts, bundled packages, complimentary services, financing incentives, and other added benefits are offered exclusively to the individual patient for whom they are issued.

Unless otherwise stated in writing, all personalized quotes, discounts, promotional pricing, bundled offers, and additional benefits remain valid for thirty (30) calendar days from the date the quote is issued, provided the required scheduling deposit for the applicable treatment is received within that thirty (30) day period.

If the required deposit is not received before the quote expires, Revive Men’s Hair Clinic reserves the right to modify pricing, promotions, discounts, financing options, treatment recommendations, or package inclusions based upon current pricing and availability.

Revive Men’s Hair Clinic reserves the sole and absolute discretion to honor any expired quote, discount, promotion, or special offer beyond its stated expiration date without creating any obligation to do so in future cases.

5.3 Financing

Financing options, including but not limited to CareCredit®, Cherry®, or other financing providers, are offered solely as a convenience to patients.

Approval decisions, repayment terms, interest rates, payment schedules, and lending decisions are determined exclusively by the financing provider.

Revive Men’s Hair Clinic LLC is not a lender and assumes no responsibility for financing approvals, denials, loan terms, payment disputes, or agreements between patients and third-party financing providers.

5.4 Stem Cell Protocol Future Hair Transplant Credit

Patients who purchase The Revive Protocol at its full regular retail price shall receive a $1,000 promotional credit toward a future NeoGraft® FUE or ARTAS® iX hair transplant procedure performed by Revive Men’s Hair Clinic.

The promotional credit:

  • Is valid for twelve (12) months from the date of The Revive Protocol treatment.
  • May be applied only toward a future hair transplant procedure performed by Revive Men’s Hair Clinic.
  • Has no cash value.
  • Is non-transferable.
  • May not be redeemed for cash or applied to prior balances.
  • May not be combined with other promotional offers unless expressly authorized in writing by Revive Men’s Hair Clinic.
  • Expires automatically if not redeemed within twelve (12) months.

5.5 Patient Satisfaction Commitment

5.5A Hair Transplant

Revive Men’s Hair Clinic is committed to providing exceptional surgical outcomes through experienced providers, advanced technology, and physician-directed care.

While every patient’s healing characteristics and biological response are unique, Revive stands behind the quality of its work.

If, following appropriate healing and clinical evaluation, Revive determines that a corrective procedure is medically appropriate, Revive may provide one (1) corrective hair transplant procedure of up to the same number of grafts originally transplanted at no additional professional treatment fee, provided the patient has:

  • Followed all pre-operative and post-operative instructions.
  • Followed all physician recommendations and prescribed medical therapy.
  • Attended all required follow-up appointments.
  • Submitted requested progress photographs within the established timeframes.
  • Promptly reported any concerns or complications.
  • Not engaged in activities that could reasonably compromise treatment outcomes.

Eligibility for any corrective treatment shall be determined solely by Revive Men’s Hair Clinic and its Medical Director.

This Patient Satisfaction Commitment:

  • Is limited to one (1) corrective procedure.
  • Covers only the professional treatment fee.
  • Does not include medications, laboratory services, travel, lodging, lost wages, or incidental expenses.
  • Has no cash value.
  • Is non-transferable.
  • Does not entitle the patient to any refund, reimbursement, reduction in fees, or other monetary compensation.

5.5B The Revive Protocol

Revive Men’s Hair Clinic is committed to delivering measurable improvements for patients who are appropriate candidates for The Revive Protocol Hair Restoration.

For patients who are determined by Revive Men’s Hair Clinic to be appropriate clinical candidates and who fully comply with the prescribed treatment protocol, The Revive Protocol is designed to achieve a minimum thirty percent (30%) increase in the appearance of hair density of existing, viable hair within approximately twelve (12) months following treatment.

Eligibility for this Patient Satisfaction Commitment requires that the patient:

  • Is determined by Revive Men’s Hair Clinic to be an appropriate clinical candidate.
  • Has existing, viable hair follicles capable of responding to treatment.
  • Completes the entire recommended treatment protocol.
  • Follows all physician recommendations, prescribed medications, and post-treatment instructions.
  • Attends all scheduled follow-up appointments.
  • Provides requested progress photographs within the established timeframes.
  • Promptly reports any concerns or complications.
  • Does not engage in activities that could reasonably compromise treatment outcomes.

At approximately twelve (12) months following treatment, Revive Men’s Hair Clinic will evaluate the patient’s results using standardized photography, physician assessment, and clinical examination.

If Revive determines that an eligible patient has not achieved the minimum expected improvement despite full compliance with all treatment requirements, Revive Men’s Hair Clinic will provide one (1) complimentary corrective treatment consisting of either:

  • One (1) additional Revive Protocol treatment

as determined by Revive Men’s Hair Clinic and its Medical Director to be the most clinically appropriate treatment.

This Patient Satisfaction Commitment:

  • Applies only to patients who satisfy all eligibility requirements.
  • Is limited to one (1) complimentary corrective treatment.
  • Covers only the professional treatment fee.
  • Does not include medications, laboratory services, travel, lodging, lost wages, or incidental expenses.
  • Has no cash value.
  • Is non-transferable.
  • Does not entitle the patient to any refund, reimbursement, reduction in fees, or other monetary compensation.

Compliance

For purposes of the Patient Satisfaction Commitment, compliance includes, but is not limited to:

  • Attendance at all required follow-up appointments.
  • Timely submission of requested progress photographs.
  • Truthful and complete medical disclosures.
  • Adherence to all prescribed medications and treatment recommendations.
  • Compliance with all pre-operative and post-operative instructions.
  • Avoidance of activities that may adversely affect healing or treatment outcomes.

Failure to substantially comply with these requirements may void eligibility for any Patient Satisfaction Commitment.

Results Evaluation

Patient outcomes will be evaluated by Revive Men’s Hair Clinic using clinical judgment together with standardized medical documentation, which may include:

  • Standardized photographs taken under substantially similar lighting conditions whenever reasonably possible.
  • Standardized camera positioning and patient positioning whenever reasonably possible.
  • Comparison of baseline and follow-up photographs.
  • Donor and recipient area evaluation.
  • Physician examination.
  • Overall clinical assessment by Revive’s Medical Director.

Results will not normally be evaluated for purposes of the Patient Satisfaction Commitment until approximately twelve (12) months following treatment unless Revive determines that an earlier evaluation is medically appropriate.

Factors Affecting Eligibility

Eligibility for the Patient Satisfaction Commitment may be reduced, suspended, or voided if the patient fails to comply with medical recommendations or if treatment outcomes are materially affected by factors outside Revive’s reasonable control, including but not limited to:

  • Continued nicotine use, smoking, vaping, chewing tobacco, or nicotine replacement products when discontinuation has been recommended.
  • Failure to follow prescribed medication instructions.
  • Discontinuation of Finasteride, Minoxidil, or other physician-recommended maintenance therapies.
  • Initiation, discontinuation, or significant dosage changes involving testosterone replacement therapy, anabolic steroids, hormone therapy, or other medications that may affect hair growth.
  • Undisclosed medications or medical conditions.
  • Chemotherapy or radiation therapy.
  • Thyroid disorders.
  • Significant hormonal disorders or endocrine abnormalities.
  • Major surgery.
  • Severe illness or hospitalization.
  • Rapid or substantial weight loss.
  • Autoimmune disorders or other medical conditions affecting hair growth.
  • Progressive or future hair loss unrelated to the treated follicles.

Corrective Treatment Scheduling

Any complimentary corrective treatment offered under this Patient Satisfaction Commitment is subject to physician approval, medical clearance, procedure room availability, staff availability, and Revive Men’s Hair Clinic’s normal scheduling procedures.

Limitation of Complimentary Treatment

Each original treatment or procedure is eligible for no more than one (1) complimentary corrective treatment.

A complimentary corrective treatment does not create eligibility for additional complimentary corrective treatments or establish any ongoing obligation by Revive Men’s Hair Clinic.

Non-Transferability

The Patient Satisfaction Commitment is personal to the original patient, has no cash value, and may not be sold, assigned, transferred, inherited, exchanged, or redeemed for cash or credit.

Revive Men’s Hair Clinic and its Medical Director shall determine eligibility in their reasonable professional judgment, compliance with the treatment protocol, clinical candidacy for corrective treatment, and whether the requirements of either Patient Satisfaction Commitment have been satisfied. The clinical determination of Revive Men’s Hair Clinic and its Medical Director shall be final.

6. Intellectual Property

All website content—including text, graphics, logos, photographs, videos, illustrations, documents, downloadable materials, website design, and software—is owned by or licensed to Revive Men’s Hair Clinic LLC and is protected by applicable copyright, trademark, and intellectual property laws.

No content may be copied, reproduced, modified, distributed, displayed, or used for commercial purposes without prior written permission.

We may also perform maintenance that temporarily limits access without prior notice.

If a dispute cannot be resolved, it shall be submitted to confidential binding arbitration in Clark County, Nevada under the rules of the American Arbitration Association. Each party shall bear its own attorneys’ fees unless otherwise awarded by the arbitrator or required by law.
