THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN ACCESS THIS INFORMATION, AND YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION. PLEASE READ IT CAREFULLY.
1. Our Legal Duties
Revive Men’s Hair Clinic LLC (“Revive,” “we,” “our,” or “us”) is required by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and other applicable federal and Nevada laws to maintain the privacy and security of your Protected Health Information (“PHI”).
We are required to:
- Maintain the privacy and security of your PHI;
- Provide you with this Notice of our legal duties and privacy practices;
- Follow the terms of the Notice currently in effect;
- Notify you if a breach of your unsecured PHI occurs as required by law.
Protected Health Information (PHI) is information that identifies you—or could reasonably identify you—and relates to your past, present, or future physical or mental health, the healthcare we provide, or payment for that care.
Examples include:
- Name
- Address
- Telephone number
- Email address
- Medical history
- Hair restoration consultation records
- Scalp photographs
- Treatment records
- Billing information
- Payment records
This Notice applies only to Protected Health Information (PHI) maintained by Revive Men’s Hair Clinic in connection with healthcare services. Information collected solely through our public website before a patient relationship is established is governed by our Privacy Policy rather than this Notice of Privacy Practices.
2. How We May Use and Disclose Your PHI Without Your Written Authorization
HIPAA permits us to use and disclose your PHI for the following purposes without obtaining your written authorization.
Treatment
We may use and disclose your PHI to provide, coordinate, and manage your healthcare.
Examples include:
- Reviewing your medical history
- Evaluating scalp photographs
- Coordinating treatment among physicians, clinicians, and staff
- Consulting outside providers or specialists involved in your care
- Ordering laboratory services when appropriate
Payment
We may use and disclose PHI to obtain payment for services provided.
Examples include:
- Processing payments
- Collecting balances
- Processing financing applications that you voluntarily submit through providers such as CareCredit®, Cherry®, or other financing partners
- Billing a health plan when applicable
Health Care Operations
We may use PHI to operate and improve our practice.
Examples include:
- Quality assessment
- Staff education and training
- Licensing and credentialing
- Scheduling
- Compliance reviews
- Internal audits
- Business planning
- Risk management
Appointment Reminders & Care Communications
We may contact you by:
- Telephone
- Text message
- Patient portal
- Postal mail
for purposes including:
- Appointment reminders
- Follow-up care
- Treatment recommendations
- Procedure preparation
- Post-operative instructions
- Billing matters
You may request that we communicate using a different method or at a different location whenever reasonably possible.
Text messages include an opt-out option by replying STOP.
Electronic Communications
At your request or with your consent, we may communicate with you by email or text message regarding appointments, treatment, billing, or other healthcare matters.
While we take reasonable precautions to protect your information, standard email and text messaging may not always be encrypted or fully secure.
By choosing these communication methods, you acknowledge and accept the associated privacy risks. You may request an alternative communication method at any time. Revive Men’s Hair Clinic will use reasonable safeguards when transmitting electronic communications but cannot guarantee the security of third-party email or mobile networks.
Individuals Involved in Your Care
With your agreement—or when you are present and do not object—or in an emergency using professional judgment, we may disclose PHI to:
- Family members
- Caregivers
- Individuals involved in payment for your care
- Persons you specifically identify
only to the extent relevant to their involvement.
Business Associates
We may disclose PHI to trusted contractors or vendors that assist us in operating our practice.
Examples include:
- Electronic Health Record (EHR) providers
- Scheduling software
- Billing companies
- Secure communication platforms
- Information technology providers
- Payment processors
Business Associates are required by federal law and written agreements to safeguard your PHI.
Business Associates may access only the minimum necessary PHI required to perform their contracted services, except where applicable law permits or requires otherwise.
Electronic Health Records
Revive Men’s Hair Clinic may maintain medical records electronically.
Electronic records are protected using administrative, technical, and physical safeguards designed to meet or exceed applicable HIPAA Privacy, Security, and Breach Notification Rule requirements, with applicable HIPAA Privacy, Security, and Breach Notification Rules.
As Required or Permitted by Law
We may disclose PHI without authorization when required or permitted by law, including:
- Public health reporting
- FDA reporting
- Abuse, neglect, or domestic violence reporting
- Health oversight activities
- Judicial or administrative proceedings
- Law enforcement requests
- Coroners and medical examiners
- Organ and tissue donation
- Approved research
- Serious threats to health or safety
- Specialized government functions
- Workers’ compensation programs
Where Nevada law provides additional protections, we follow the more protective law.
3. Uses and Disclosures Requiring Your Written Authorization
Except as described above, we will obtain your written authorization before using or disclosing your PHI.
This includes:
- Marketing communications requiring authorization under HIPAA
- Any sale of PHI (Revive does not sell PHI)
- Before-and-after photographs
- Patient testimonials
- Social media posts
- Website photographs
- Promotional videos
- Any identifiable treatment images
- Most uses or disclosures of psychotherapy notes, if any are ever created
- Any other use not specifically permitted by law
Providing authorization for marketing or photographs is completely voluntary and is never required to receive treatment.
You may revoke an authorization at any time by submitting a written request.
Revocation does not affect actions already taken in reliance upon your authorization.
4. Your Rights Regarding Your PHI
You have the following rights.
Right to Inspect and Obtain Copies
You may inspect or receive copies of your medical or billing records in paper or electronic form when readily producible.
Reasonable cost-based copying fees permitted by law may apply.
Right to Request an Amendment
If you believe your record is inaccurate or incomplete, you may request an amendment.
If we deny your request, we will explain the reason in writing and inform you of your right to submit a statement of disagreement.
Right to an Accounting of Disclosures
You may request a list of certain disclosures made during the previous six years.
The first accounting within any twelve-month period is provided at no charge.
Right to Request Restrictions
You may request restrictions on our use or disclosure of your PHI.
Although we are not required to agree to every request, we must honor requests restricting disclosure to a health plan if:
- You paid in full out-of-pocket; and
- The disclosure relates solely to that service; and
- Disclosure is not otherwise required by law.
Right to Confidential Communications
You may request communications through:
- A different telephone number
- A different mailing address
- A different email address
- Another reasonable communication method
We will accommodate reasonable requests whenever possible.
Right to Designate a Personal Representative
Subject to applicable law, you may authorize another individual to act on your behalf regarding your Protected Health Information.
We may require documentation verifying that person’s authority before releasing information, unless prohibited by applicable law.
Right to Breach Notification
You have the right to receive notification if a breach involving your unsecured PHI occurs.
Right to Receive a Paper Copy
You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
The current Notice is always available at our clinic and on our website.

5. Changes to This Notice
We reserve the right to revise this Notice at any time.
Revisions may apply to all PHI we maintain, including information created before the revision.
Updated versions will be posted at our clinic and on our website together with the revised Effective Date.
Availability of This Notice
A current copy of this Notice is available upon request at our clinic and is posted on our website. Patients may request a printed copy at any time without charge.
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with Revive Men’s Hair Clinic or with the U.S. Department of Health and Human Services, Office for Civil Rights.

You will not be retaliated against or penalized in any way for filing a complaint.
